We carry out revalidation or renewal UK class 3 (ATCO) medicals for existing medical certificate holders at at either our Birmingham Airport or Harpenden (near Luton Airport) clinics.

A UK CAA Cellma online application form (if applying for a UK CAA medical. You do not need to bring a printed copy)
Your completed mental health questionnaire (do not date or sign)
Your glasses or contact lenses, if you wear them
Your medical reports relating to any significant current or past medical history

Bring a completed eye exam form from an optometrist if not submitted in the past 2 years. Use:
